Intra-operative MR guidance during trans-sphenoidal pituitary resection: preliminary results.

چکیده
The use of intra-operative MR image guidance has the potential to improve the precision, extent, and safety of trans-sphenoidal pituitary resections. The trans-sphenoidal approach to pituitary surgery has been performed for some time (1--3). Until now these surgeries have relied on direct visualization without the aid of image guidance. An open-bore configuration 0.5T SIGNA SP MR system (GE Medical Systems, Milwaukee, Wisconsin) has been used to provide image guidance for seventeen trans-sphenoidal pituitary adenoma resections (4). The intra-operative MRI system allowed the radiologist to successfully direct the surgeon toward the sella turcica while avoiding the cavernous sinus, optic chiasm and other critical structures. Imaging performed during the surgery monitored the extent of resection and allowed for removal of tumor beyond the surgeon's view in seven cases. Dynamic MR imaging was used to distinguish residual tumor from normal gland and postoperative changes, permitting more precise tumor localization. A heme-sensitive long TE gradient echo sequence was used to find the presence of hemorrhagic debris. All patients tolerated the procedure well without significant complications. J. Magn. Reson. Mononostril versus binostril versus one and a half Nostril Endoscopic Trans sphenoidal approach for pituitary adenoma
Background and Aim: Over the past 2 decades, endoscopy has become an integral part of the surgical repertoire for pituitary procedures and over the past several decades, the endoscopic endonasal trans sphenoidal approach (EETA) has gradually become an accepted option for pituitary tumors with intra-,supra-,and para-sellar location and extension, because of its minimal invasiveness and high effi...
متن کاملVolumetric measurement for comparison of the accuracy between intraoperative CT and postoperative MR imaging in pituitary adenoma surgery.
BACKGROUND AND PURPOSE To improve the resection rate of unexpected residual pituitary tumor under image guidance, iCT provides a less time-consuming and more convenient approach of promising the safety of the trans-sphenoidal surgery. However, iCT was thought to have worse image quality than MR imaging. This study was designed to determine the predictive concordance of iCT with standard postope...
متن کاملFeasibility of Endoscopic Endonasal Approach for Recurrent Pituitary Adenomas after Microscopic Trans-Sphenoidal Approach
OBJECTIVE The surgical approach for recurrent pituitary adenoma after trans-sphenoidal approach (TSA) is challenging. We report the outcomes of the endoscopic TSA for recurrent pituitary adenoma after microscopic TSA. METHODS From February 2010 to February 2013, endoscopic TSA was performed for removal of 30 recurrent pituitary adenomas after microscopic TSA. Twenty-seven (90%) patients had a...
متن کاملManagement of intra-operative cerebrospinal fluid leak following endoscopic trans-sphenoidal pituitary surgery.
OBJECTIVE Cerebrospinal fluid leakage is the most common complication of endoscopic trans-sphenoidal pituitary surgery. However, there is no uniformly accepted way of managing this complication when it occurs intra-operatively. This paper describes a quick, simple technique, involving layered fibrin glue and gelatin sponge, which does not compromise post-operative patient follow up. METHOD Re...

INTRODUCTION Endoscopic, transnasal management of pituitary gland neoplasms is a widely accepted alternative to the traditional microscopic approach. This study aimed to determine outcomes and complication rates for the largest UK series of endoscopic, trans-sphenoidal hypophysectomies reported to date. METHODS We performed a retrospective analysis of 136 primary resections and 35 revision ca...
